Microcosm is a pseudo-3D shoot them up. The miniature ship progresses through the vascular system and then the president's cerebral cortex, facing waves of ever more dangerous enemies. The player can sometimes choose their path at certain branches, bringing a semblance of strategy to an otherwise rather classic and linear game system.

The precalculated decorations, very realistic for the time, were produced on a Silicon Graphics workstation and adapted with a more or less high quality according to the capacities of the machines. The game is especially valuable for its atmosphere and presentation. Some game music was composed by Rick Wakeman, one of the keyboardists of the group Yes.

Plot

The game takes place in a cyberpunk universe. The president of a major corporation has been infected with a droid virus from the rival multinational. An agent is miniaturized and injected into the president's body to eradicate the threat.
