The game takes place in Egypt and Rome, it has a different graphic style than the other games in the series, but several enemies and armaments return from The First Encounter and The Second Encounter.
The game introduces several new, exclusive enemies and weapons. The story revolves around Sam going back in time again, as a group of people studying ancient civilizations on Earth encounter new forces from the hybrid army of Mental, the series' antagonist. Sam has to return to Egypt first, and then proceed to Rome, facing hordes of enemies on the way. The game bosses (each faced at the end of each region) are two similar commanders with a lycanthropic appearance: SYrian Sphinx (Egypt) and Wolfiator (Rome). With the latter's defeat, Sam Stone, again victorious, returns to his time.
